## Narrative

On September 28, 2021, at approximately 5:30 a.m., an employee separated two paper rolls and began feeding the first paper roll into a die cutting press used to fabricate file folders. Each roll weighs approximately 900 pounds. The second roll tipped over and struck the employee's left leg, resulting in a fractured femur.
