## Narrative

An employee was adjusting a ram on a filter press by pulling on a press plate with a hook. The hook slipped, causing the employee to slip and fall 6 feet from an elevated platform. The employee struck the staircase and the floor. The employee suffered two broken wrists, a broken nose, and a concussion. The employee was hospitalized.
