## Narrative

An employee was in the process of removing a label from a combo. The employee moved to get out of the way of a forklift when the rear of the forklift contacted the combo and the employee's left hand was caught between the combo and the forklift. The employee suffered an open fracture to the distal left phalanx and to the thumb.
