## Narrative

An employee was climbing on the back of a spray truck. The employee's legs became entangled in a hose, causing the employee to fall from the bed of the truck to the ground about 4 feet below. The employee suffered two broken ribs, a concussion, and bruises to the right shoulder and left foot.
