## Narrative

An employee cleared a jam in the heat set on a machine. The employee removed the lock out device and energized the machine. The employee then removed the guard to adjust a belt. The belt started moving and when the employee reached for a tool his right hand was caught in the heat set roller, resulting in a partial amputation of the right index finger.
