## Narrative

An employee was removing a small scaffold that had been erected for cleaning in a conical hopper. The employee had handed up the last piece of material and was positioning himself to exit the hopper when his foot slipped. He slid at an angle and his right leg was caught, twisted, and broken above the ankle.
