## Narrative

Employees were removing unions from pipe in a boiler room. One employee was holding a ladder for another employee. One of the unions had loosened through many cycles of heating and cooling, though the employees had not touched it yet; it failed to hold the weight of an 18-inch length of 2-inch pipe, which fell onto the first employee's head. The employee suffered a severe head laceration.
