## Narrative

An employee in a warehouse was helping a forklift operator retrieve an item from a rack. The forklift was moving forward and slowly coming to a stop when the employee started to step up onto the forklift to communicate with the operator. The forklift ran over the employee's left foot and the big toe was fractured.
