## Narrative

On February 22, 2022, at 9:51 p.m., a maintenance employee was removing clogged material from a chip feed hopper when an operator in the control room turned on the machine. The valve rotated and the employee's hand was caught by the machine, resulting in finger amputations. The machine was not locked out or tagged out at the time.
