## Narrative

An employee was trying to retrieve a magnet from inside a powered conveyor. The conveyor arm dropped down and crushed the employee's right arm against the conveyor's frame. The employee suffered a torn artery and muscles in the arm and was hospitalized, requiring surgery. The machine was not locked out/tagged out at the time of the incident.
