## Narrative

An employee was changing a pinion gear in the gearhead that drives a vertical turning lathe. The gearhead fell and the employee stepped out of its path and fell off the round worktable onto the floor. The employee sustained a dislocated and fractured left shoulder, a fractured left forearm, and contusions on the left elbow.
