## Narrative

An employee was working on the main hydraulic "pipe" of a forklift, whose carriage had been raised about 4 feet so the employee could do the work. The carriage fell and struck the employee, who suffered a head laceration, a dislocated shoulder, and a broken leg. The employee was hospitalized.
