## Narrative

On August 19, 2022, an employee was using a forklift when his shoelace became caught in racking. While trying to free his shoelace from the racking, his foot became caught between the equipment and the rack's bottom protection barrier. The employee sustained tissue damage to his foot, which required surgery to repair.
