## Narrative

On September 9, 2022, at 1:20 a.m., an employee emptied a trailer by lifting it with a hoist. The employee lowered the trailer and went to remove the hook from the connection point on the trailer. As they put their left hand on the hook, the hoist activated, causing their fingers to get crushed between the hook and a metal bar on the trailer. The employee suffered crush injuries to their middle and ring fingers, resulting in surgical amputation down to the first knuckle.
