## Narrative

An employee was placing a brick under a steel plate to make room for a shear with a fork attachment to lift the plate and place it onto a truck skid. The weight of other stacked steel plates caused the plate to fall and crush the employee's right index finger. The employee suffered an amputation to the finger.
