## Narrative

At 1:18 p.m. on February 2, 2023, employees were moving an 8-by-10-foot road plate, weighing about 3,000 pounds, using a front loader with a fork attachment. The fork was being used to raise one end of the plate so that dunnage could be inserted between it and the plate it was stacked on. The plate slid off the fork while it was 6 inches off the ground and struck the front of an employee s left foot. He suffered a displaced fracture to the last joint of his left big toe.
