## Narrative

As an employee was leaving their work station to go on break, they noticed that paper on a roller that was approximately 5 feet off the ground had folded over. As the employee attempted to fix the roller, their hand became caught between two rollers, resulting in a fingertip amputation and the loss of a fingernail.
