## Narrative

On February 20, 2023, an employee was watching the backhoe move steel plates for storage when the employee was instructed to attach a 4,000-pound chain to the excavator's bucket. The other end of the chain was looped through the steel plate's hole. While the employee was attaching the chain to the excavator bucket, his left ring finger became caught between the chain and the bucket, resulting in the amputation of his left ring finger.
