## Narrative

Two employees were cleaning the end of an 8-inch PVC pipe (approximately 15 feet long) to set it in a manhole using a crowbar. As the injured employee pushed the pipe in, three of his left-hand fingers were pinched, resulting in an injury that required surgery.
