## Narrative

An employee was working on a shrink wrap machine and placed a pallet on the turntable. The shrink wrap film was held down so the film would engage the corners of the pallet. The employee's left middle finger was caught between the rotating turntable and the porch panel, resulting in an amputation to her left middle finger.
