## Narrative

On June 2, 2023, at approximately 1:00 a.m., an employee was placing air in a tire. The rim was damaged on the inside due to bearing failure, which caused the tire to come unseated. The tire, rim, and hub blew off from the axle, striking the employee in both legs. The employee sustained a compound fracture to the left leg and a broken right foot; both required hospitalization and surgery.
