## Narrative

An employee was preparing to pump chemicals from a tote on a truck bed into a continuous injection tank. While walking on the tote and transitioning to another container, the employee slipped/tripped and fell to the ground sustaining fractures to vertebrae and the left wrist and a head laceration.
