## Narrative

An employee was assisting with removing material from a roof using a derrick. The employee was standing in the bed of a truck while others were hoisting an empty gang box into the bed of the truck. A piece of the derrick came loose and struck the employee in the head. He was hospitalized with a skull fracture.
