## Narrative

An employee was preparing to pour a precast concrete product. They used a ladder to climb on top of a main casting form to position a ring section. The ring was being held over the top of the casting form by an overhead crane. As the employee positioned the ring, one of the supporting chains connecting it to the crane came loose. This caused the ring to knock the employee off the casting form. The employee fell approximately 9 feet 2 inches to the ground below and sustained a head contusion and fractures to their scapula, back, and ribs.
