## Narrative

An employee was fixing a loose rod on a forklift. His thumb was on the inner portion of the forklift between the mast and hydraulic cylinder. The employee asked a co-worker to move the forks and the hydraulic cylinder crushed his right thumb, resulting in a partial amputation.
