## Narrative

On November 1, 2023, four employees had just adjusted a conveyor belt from the top of a grain bin. The conveyor belt was then energized and caught an employee's right arm/hand and caused a fractured/dislocated right ulna, tears to tendons in the forearm, and required a skin graft.
