## Narrative

An employee was deep scrubbing the floor using a low-speed floor scrubber machine. The cord for the scrubber got wrapped around the machine head and caused it to jerk suddenly. This caused the employee to lose his footing on the wet floor and fall, resulting in a fracture to his left tibial plateau that required surgery.
