An employee was retrieving rigging straps to begin rigging a stack of wooden and concrete forms to be lifted via an excavator boom off a bridge deck. While retrieving the straps, he stepped between the stack of forms and the excavator boom. At this time, the excavator shifted, causing the boom to shift and strike him in the head and neck area. The employee suffered a nondisplaced fracture to the jaw, a fracture on the right facial orbital area, and a neck laceration on his left side.
