## Narrative

An employee was pushing a cart carrying a steel part, which was being held down by a cylinder block. The steel part's momentum shifted, causing the cylinder block to slide toward an insert on the cart. The employee's hand was over the insert, and the block amputated the tip of the employee's thumb.
