## Narrative

An employee was using a lift device to place a pallet jack onto a block to repair it. The pallet jack was not fitting on the block and the employee attempted to reposition it when the pallet jack slipped and pinched his left middle finger against the wood block. The employee suffered a fingertip amputation.
