## Narrative

An employee was on a forklift picking orders of conduit reels to load on an 18-wheel flatbed truck. As he loaded reels onto the truck, he identified two reels needed for a shipment in a row of ten. He rolled the two reels out by hand in order to reach them with the forklift. As he rolled one of the reels out, the reel rolled back into the row. He pulled the reel back out and went to hold it in place (chock) with a bolt when his left index finger was caught between the reel and the concrete ground, resulting in a fingertip amputation below the fingernail.
