## Narrative

An employee was harvesting mushrooms from his mushroom picking aid (MPA). The basket of the MPA disconnected from the mast and fell forward to the ground. The employee was inside the basket and sustained injury to their right shoulder and struck their head on impact. The employee was hospitalized.
