## Narrative

An employee descended a step ladder to the top of a boxing fixture that was 1-2 feet off the ground when he fell off the boxing fixture to the concrete floor, resulting in a fractured right wrist. He lost traction on the fixture due to the presence of grinding dust on the metal surface.
