## Narrative

An employee was working with a teammate to separate the top and bottom sections of a steel die set. The top section was chained to a lift and was sitting on steel saw horses. The left corner of the top section became stuck, causing the saw horse to shift and the die set fell onto the employee's feet, resulting in four fractured bones in their right foot and a fracture in their left big toe.
