## Narrative

On May 10th, Employee 1 was operating the depositor when their left middle fingertip was amputated by the hopper. 
On May 18th, Employee 2 had been wiping a product spill from a conveyor. When completed, the line was activated and the employee's hand was caught in a pinch point, amputating their fingertip.
