## Narrative

An employee was in a building to perform maintenance work on a boiler, chiller, and HVAC system. Looking for roof access, the employee opened a set of double doors on the third floor that opened outward toward a maintenance shaft/elevator shaft. He then looked for a light switch to turn on and stepped forward through the doors. He fell approximately 15-20 feet down the shaft, landing on his lower back on the trap door . The employee sustained a laceration to the back of their left leg that required stitches.
