## Narrative

An employee was operating a panel saw to cut cardboard for shipping containers when the blade became jammed on the cardboard. The employee was unjamming the saw when the blade contacted her left hand and she sustained a partial amputation to her thumb and two of her fingers were lacerated. The saw was not locked out/tagged out at the time.
