## Narrative

An employee was moving a header using a forklift for a scheduled training session. The header rolled over the tire chocks and the employee was pinned between the header and the backrest of the forklift. He freed himself and then fell to the ground. The employee was hospitalized for a fractured sternum, fractured ribs, and punctured lungs.
