## Narrative

On September 4, 2024, an employee was using a poke rod to clear a jam from a floor grate that dried distillers grain (DDG) is loaded into for a conveyor. He fell backward to the floor and sustained a brain bleed, two rib fractures, and fractures to their T1 and T2 vertebrae.
