## Narrative

On October 4, 2024, an employee was mixing waffle batter using a drill mixer and a cover with a hole in it to allow the mixer to enter the bowl. The employee attempted to cover the hole when the mixer contacted their right thumb, resulting in an amputation to the thumb tip.
