## Narrative

An employee was standing on top of counterweights that were staged at ground level to hook them up to a crane hook. He moved backward and stepped into a rung on the side of the counterweights. He fell backward off the counterweights while his foot was still in the rung, resulting in fractures to his tibia and fibula above the ankle, which required surgery.
