## Narrative

An employee was installing a fan in a dairy stall. They were standing on a 4-by-6-foot wooden pallet with a solid top that was lifted on a pallet fork attached to a loader. The bottom brace board on the pallet broke, causing the pallet to flip forward. The employee subsequently fell forward onto a pipe railing, resulting in internal traumatic injuries to their pelvic and abdominal areas.
