## Narrative

On 2/27/2025, at 11:55 AM, an employee was changing an insert plate on a hydraulic compression mold. The employee reached under the top mold and his head contacted the the hydraulic lever, causing the top to come down on the employee's arm. The employee required surgery to set the broken bone.
