## Narrative

On March 18, 2025, an employee was working from an 8-foot A-frame ladder while using a battery-powered impact wrench to break bolts loose from a pipe in the ceiling. As they descended the ladder, the employee fell to the ground, resulting in a brain bleed and cut/scrapes on their hands.
