Per row: state, county, 11-digit tract GEOID, category (Health Outcomes / Risk Behaviors / Prevention / Disability), measure (e.g. BPHIGH high blood pressure, OBESITY, MAMMOUSE mammogram use, DEPRESSION, etc), crude and age-adjusted prevalence %, 95% confidence intervals, total population. Companion to cdc_places_county (county-level we already have). Joins via county_fips with cms_hospital_service_area, fema_disasters, FQHC enrollments.
